Support at Home starts with your life.

Support at Home is the Australian Government program for eligible older people who need coordinated help to remain at home. It began on 1 November 2025, replacing Home Care Packages and Short-Term Restorative Care. Your aged care assessment determines the services and funding you are approved to receive.

How do I access Support at Home?

Eligibility and approved services are determined through an aged care assessment. You can apply through My Aged Care online or by calling 1800 200 422.

Does Support at Home pay for allied health?

Allied health may be included as a clinical support when it is approved in your support plan. Your registered provider should confirm the available funding and service arrangement before therapy begins.

Will I pay a contribution for allied health?

Under current Support at Home settings, participants do not contribute to clinical support costs. Contributions can apply to independence and everyday living services and depend on individual circumstances.

Who coordinates my Support at Home services?

Under the single-provider model, the older person’s registered provider oversees the service agreement, care management and delivery of approved services. Bloom can discuss an allied health service arrangement with that provider.

What are the short-term pathways?

Support at Home includes Restorative Care, Assistive Technology and Home Modifications, and End-of-Life pathways. Approval depends on the person’s assessment and circumstances.

What information should a provider include in a referral?

Include the person’s consent, postcode, approved service, goals, relevant health and risk information, preferred setting, funding contact and any required reporting timeframe.

What is Support at Home?

Support at Home is Australia's government-funded aged care program designed to help older people remain independent, safe and well in their own homes for as long as possible. It replaced the Home Care Packages Program and Short-Term Restorative Care Programme on 1 November 2025.

What happened to the Home Care Packages Program?

The Home Care Packages Program, along with the Short-Term Restorative Care Programme, was replaced by Support at Home on 1 November 2025.

Is the Commonwealth Home Support Programme (CHSP) still available?

Yes, the Commonwealth Home Support Programme (CHSP) continues to operate separately from Support at Home and is expected to transition no earlier than 1 July 2027.

How is the Support at Home program structured for funding?

Support at Home features a flexible funding framework comprising 8 main funding classifications for ongoing services, 3 short-term pathways (for assistive technology and home modifications, restorative care, and end-of-life care), and 4 transitioned Home Care Package classifications to provide continuity for those who previously received Home Care Packages.

What are the main service categories under Support at Home?

Support at Home services are organized into three main categories: Clinical Supports, Independence Services, and Everyday Living Services.

What does 'Clinical Supports' include under Support at Home, and who pays for it?

Clinical Supports help maintain health, function, safety and wellbeing and are fully funded by the government. They include allied health services such as physiotherapy, occupational therapy, speech pathology, exercise physiology, and psychology, as well as nursing services and wound care and clinical interventions.

What kinds of services fall under 'Independence Services'?

Independence Services help older people manage everyday activities and stay connected with their community, and may include personal care and daily support, transport for medical appointments and social outings, respite care, and assistance with daily routines.
