Where can appointments take place?Bloom primarily provides support at home and in the community. Telehealth may be available when it is clinically suitable. Clinic appointments may be available case by case.
Can a family member or carer be involved?Yes. With the older person’s consent, family members, carers and other trusted supporters can join conversations and help put strategies into practice.
Can Bloom work with my existing aged care provider?Yes. Bloom can work with older people, families and registered Support at Home providers. The funding and service arrangement must be confirmed before services begin.
